THIS IS AN IMPORTANT DOCUMENT AND MUST BE READ AND UNDERSTOOD BEFORE SIGNING
Before participating at Kidstown Corbally, Kidstown Delta and Splatown all participants must register to play by completing the waiver/risk acknowledgement form below.
Participants under the age of 18 must have the waiver/risk acknowledgement completed by a parent or legal guardian
I wish to participate in indoor play activities including, but not limited to trampolining, volcano climbing, wall climbing, fitness classes, dodgeball, aeroball, basketball, foam pit, airbag, walk the wall, performance trampoline, battle beam, jump tower, slack line, assault course, bouldering wall, inflatable assault course, inflatable slide (collectively hereinafter called ”the Activities”) organised by Kidstown Ltd (”the Company”) within 12 months of the date of completion of this form.
I am eighteen years old or older.
I agree that I will undertake the Activities in accordance with the safety rules made available to me prior to participation (which can be seen on our website at www.kidstown.ie and the advice given in the pre-participation safety briefing to be undertaken prior to participation, together with any oral instructions or advice given to me prior to or during the session.
I agree to wear Kidstown anti-slip grip socks whilst participating in the trampoline Activities. I confirm that if I am using pre-used Kidstown anti-slip grip socks that they are in a good condition and fit for purpose and acknowledge that Kidstown Ltd cannot be held responsible for the efficacy of these anti-slip grip socks.
I agree to wear socks whilst participating in the soft play activities. I confirm that if I do not wear socks I acknowledge that Kidstown Ltd cannot be held responsible
I acknowledge that I am responsible for my own safety (and the safety of my possessions) while undertaking the Activities.
I certify that to the best of my knowledge that I am in good physical condition and I do not have any medical condition which might have the effect of making it more likely that I will be involved in an incident which could result in injury to myself or others.
I certify that I have no pre-existing medical condition which, could through participation in the Activities, result in injury to myself.
I certify that to the best of my knowledge I am not pregnant
I acknowledge that the maximum permitted weight for participation in the Activities is 122kg / 19 stone and 3 pounds and confirm that I do not exceed this weight.
I understand that the Activities provided are not segregated for use solely by children. I understand that the Activities are used by multiple age groups including adults at the same time as children and are not restricted to any one age group.
I acknowledge that it is my responsibility to assess the suitability of the Activities & the multiple age group use for younger children in my care. I am aware that certain activities are age and or ability restricted and I will ensure that the child/ren in my care is/are advised accordingly.

I am the parent/guardian of the child/ren listed below who is/are under 18 years of age. I consent to allow that/those child/ren to participate in indoor play activities including, but not limited to trampolining, volcano climbing, wall climbing, fitness classes, dodgeball, aeroball, basketball, foam pit, airbag, walk the wall, performance trampoline, battle beam, jump tower, slack line, assault course, bouldering wall, inflatable assault course, inflatable slide (collectively hereinafter called ”the Activities”) organised by Kidstown Ltd (”the Company”) within 12 months of the date of completion of this form.
I declare that if I am not the parent or guardian of the child/ren I have authority from the child/ren’s parent or guardian to sign this risk acknowledgement & waiver form.
I acknowledge the risk on their behalf.
I confirm that I am responsible for the child/ren in my care and undertake to ensure that he/she/they undertake the Activities in accordance with the safety rules made available prior to participation (which can be seen on our website at www.kidstown.ie and advice that I and he/she/they will be given in the pre-participation safety briefing to be undertaken prior to participation, together with any oral instructions or advice given to me and he/she/them prior to or during the session.

I acknowledge that I am responsible for the safety & supervision of the child/ren named below (and the safety of our possessions) and that Kidstown Ltd do not provide supervision of children whilst using the premises or if left unattended on the premises. I will ensure that I pay particular attention to any under 16 year olds and will supervise them at all times.
I certify that to the best of my knowledge the child/ren is/are in good physical condition and do not have any medical condition which might have the effect of making it more likely that he/she/they be involved in an incident which could result in injury to him/her/themselves or others (if in any doubt, please check with the child/ren’s parent or guardian).
I certify that the child/ren have no pre-existing medical condition which, could through participation in the Activities, result in injury to them.

I agree that I will not attempt to use any of the activity equipment or participate in the Activities whilst accompanying the child/ren unless:
I am a registered participant;
I have signed a risk acknowledgement & waiver form as a participant in the Activities;
I am aware of and have agreed to undertake the Activities in accordance with the safety rules made available to me prior to participation (which can be seen on our website at www.kidstown.ie and the advice given in the pre-participation safety briefing to be undertaken prior to participation, together with any oral instructions or advice given to me prior to or during the session.

I understand that trampoline parks are made up of single trampolines and interconnected trampolines that are not enclosed and are without segregation or barriers to allow for the free movement around the court between trampolines.
I understand that the Activities are used by multiple age groups at the same time and are not restricted to any one age group.
I understand that the Activities are physically demanding and involves jumping and other strenuous actions sometimes involving height, speed, and unpredictable surfaces.
I understand that the Activities are a high-impact, full body activity which requires intense focus, awareness of my body’s strengths and limitations, awareness of the environment around me, and extreme caution at all times.
I acknowledge that the Activities are dangerous and there is a risk of personal injury when undertaking such Activities and participation can result in death or serious injury.
I acknowledge that participation in the Activities can result in personal injury including, but not limited to, friction burns, cuts, abrasions, bruising, muscle strains, twists, sprains, dislocation, broken bones, paralysis and accept that these injuries can occur through general use of the equipment and not always or necessarily as a result of defective equipment or negligence of the Company.
I acknowledge and accept that the Activities require a moderate level of fitness and can be physically demanding and require physical effort and I/he/she/ they should not undertake the Activities unless I/he/she/they am(is/are) physically able to.
In the unlikely event of an accident, or loss or damage to any personal effects, I acknowledge that the Company will not be liable for any direct or indirect loss, damage or injury arising from or in connection with the Activities (except for death or personal injury caused by the Company’s negligence) and I waive all and any claims against the Company in this respect.
I acknowledge and accept that CCTV images of all park entrants will be monitored and recorded throughout the premises for the purposes of crime prevention and public safety.
Trampoline Park Rules
- Jumping on a trampoline can be great fun but it can also be dangerous. There is a risk of serious injury if care is not taken when using the trampolines. Using trampolines is physically demanding and involves jumping and other strenuous actions involving height, speed and unpredictable surfaces. Jumping on trampolines is a high-impact, full body activity which requires intense focus, awareness of your body’s strengths and limitations, awareness of the environment around you and extreme caution at all times
- Only jump if you are in good physical condition
- Do not jump if you have any pre-existing medical conditions including heart, back, neck, bone or muscle conditions
- Do not jump if you are or may be pregnant
- Do not attempt to use any of the activity equipment unless you have registered to participate, undertaken the safety briefing & signed a waiver (if you are under 18 a waiver must be signed by a parent or guardian)
- Before jumping loosen up with warm up exercises and stretches
- If you get tired please leave the arena and take a break
- Always remain in control – Be aware of how to bend your knees to control or stop your bounce
- Practice first before moving around the arena
- Always plan your landings before jumping
- Always jump & land in the centre of the trampolines
- Never jump on or near the coloured pads
- Do not exit the trampolines by ‘bouncing’ off onto the surrounding walkways
- Make sure you have control before leaving the trampolines
- Always jump so you land on both feet at the same time. Never land with just one foot
- Know your limits – Do not attempt any move beyond your own skill level
- Never attempt flips or other extreme moves unless you know you are capable and have had the appropriate training
- No double flips or triple flips even if you are capable
- No double bouncing! (There should never be two people on one trampoline at the same time)
- Strictly only one person per trampoline at any one time
- Do not jump or step onto a trampoline unless the trampoline is clear
- Look out for other jumpers and give way to smaller ones
- Never run across the trampolines
- No diving, No backflips, No gainers (forward moving backflips)
- No wrestling, tackling, pushing or shoving
- No climbing the trampolines or the padding
- No leaning, grabbing or climbing on any netting or the basketball hoops
- No sitting or lying on the trampolines or the padding
- Remove all items from your pockets prior to jumping
- All jewellery, watches, keys, keychains, coins or the alike to be removed prior to jumping
- Clothing should not have any hard or sharp points (such as buckles, studs, toggles)
- No phones or cameras on the jump arena
- No food, drink or chewing gum whilst jumping
- Do not leave clothing on the jump arena, use the lockers provided
- Approved Air-tastic anti-slip socks must be worn at all times
- No jumping under the influence of alcohol or drugs
- Always follow the marshals instructions. Dangerous behavior will not be tolerated and you could be asked to leave
- We do not provide or accept responsibility for the supervision of children
- Children must be supervised by a parent or guardian at all times
- Under 5’s not allowed in the main area
- Maximum weight restriction is 122Kg (19 stone 3 pounds)
Soft Play Rules
- Children must be supervised by a parent or adult at all times in the play area
- Main Play equipment is designed for children ages 5-8.
- Toddler Area equipment is designed for children under 3 years.
- Adults are NOT permitted on the play equipment.
- Please remove shoes, loose articles of clothing and jewellery. Place shoes in shoe holders before using the play equipment.
- Socks and long sleeves must be worn at all times.
- Slide one at a time in a forward seated position, feet first.
- Please no climbing on or up the slides.
- Please no climbing or standing on the outside of the play equipment.
- Please no running, pushing, hitting or biting allowed.
- Please no food, gum or drinks in the play equipment.
- If your child has a medical condition you must inform us before entering.
- Kidstown accepts no liability for supervision, loss, damage or injury to persons or property within the centre, this does not affect you statuary rights.
- Height and age restrictions are in force for the safety of all the children, please respect them.
Climbing Wall Safety Rules
- All participants (boulderers, belayers, and climbers) must wear closed-toed shoes – no bare feet or open-toed sandals are permitted
- All boulderers must wear a helmet at all times they are on the wall
- All belayers must wear a harness and be connected to an anchor while belaying
- All climbers must wear a helmet and harness and must be on belay at all times they are on the wall
I have read and accept the rules of play. I understand it is a play at your own risk establishment and that all injuiries or accidents are the responsibility of the parents/guardian. I release Kidstown play centre and all associates from liability of accident.
